COMPLETION OF THIS FORM IS REQUESTED FOR FULL CONSIDERATION; RETURN WITH AN

POSITION. RETURN TO:

The following question provides additional information regarding your
experience, skills, knowledge & abilities relating to the above mentioned
position. Please answer as accurately & specifically as possible. While you
may
attach additional pages, as necessary, we encourage you to be concise in your
responses.

Please respond to the following hypothetical situation:

During a routine inspection of a laboratory, you notice that the monitoring
log
records have not been completed for the past two months. (At UCSC, monitoring
logs must be completed on a bi-weekly basis). You also find some minor
unlabeled contamination in the radiation-use areas on bench tops, pipetters,
and a sponge.

When you confront a laboratory member with these findings she indicates that:

"Everyone monitors the lab after their own use. We have a rotation schedule
for
the entire lab and someone is supposed to monitor it every week. I know we did
it last week. We just probably forgot to record the information."

How would you handle this situation? In responding, please address these
points: What do you hope to accomplish? What are the important issues? What
methods would you use to convey the important issues? What things would you be
mindful of when dealing with the laboratory member? What next course of action
would you recommend to the laboratory member?

If you are unable to fill out the form due to a disability or other special
circumstances, please call the Staff Human Resources Office (408) 459-2011
PRIOR to the final filing date.
