Laser Eye Medical Exams

For those individuals who also function as the Laser Safety Officer.
What type of medical exam is required at your facility as a baseline for
laser users ?

I am aware of the requirements specified in ANSI Z-136.1-1993 6.1, that
3B and 4 class users shall have medical surveillance.  6.3.2 states that
laser personnel shall have the following exams:

1. Ocular history reviewed
2. visual acuity
3. Amsler Grid
4. Color Vision.

Do your personnel have all of these exams?  If not what is your concern
that you may have a liability issue by not having all personnel who use
lasers have a baseline exam with at least the 4 items above ?   Do you
require a termination exam when personnel leave employment ?
