Re: DOT form
There is no generic DOT form. The NRC has a uniform radioactive waste
manifest, but its use is limited to radwaste shipments. Your standard
company
bill of lading is probably adequate, if you include the information required
by 49 CFR 172, and attach any required characterization information.
Before you do this, however, you should be aware that this makes you, and
anyone else involved with the shipment, a hazmat employee. All hazmat
employees require hazmat employee training, as specified in 49 CFR 172,
Subpart H. Your activies may also require registration with DOT, as
specified
in 49 CFR 107, Subpart G. If your organization does not have a qualified
hazmat shipper, you should hire a consultant.
