Required State approval - dose changes

I am aware that both IL and TX require the state approval before a 
reported dose can be changed. CA used to require this, but that 
requirement ended about 3 years ago (noted per a recent phone 
conversation). I would like to know what other states require prior 
approval by their office before a dose record can be changed.
