As Kathleen stated above, this requirement most likely will be re-
instituted. In my recent phone conversation, the original intent was 
that the state required review of any dose that exceeded regulatory 
limits, prior to changing, and not a change to any reported dose. I 
hope that this intent is followed though on, and, the wording is 
straight forward where the intent is understood by all.
