In response to your question, the items you listed above are in 
essence, what I was referring to. Radiation safety is a requirement. 
The degree of what actions are to be taken to ensure radiation 
safety, should be based on the risks at hand, the history of 
problems within the facility, the competency of the staff, etc. What 
Paul Lavely is doing at Berkely summarizes what I believe a well-
run program should be based on. Accountabilities based on facts, 
and not just perceptions.
