We don't use a point system per se, but have adopted the "three strikes" 
policy, promulgated by a certain criminal justice school of thought.  ( No 
violation defined for "research under the influence, yet!" )   Anyway, we 
track violations (as noted during our lab audits)  by laboratory and 
authorized user.  A violation notice is sent following the first volation 
(i.e. EDS, lack of RAM security, excess
contamination, etc..).  If the same problem is noted a second time, the AU 
is again notfied and a meeting with him/her is held in order to help 
resolve the problem. If a third violation occurs within a 12 month period, 
the issue is brought to the Radiation Safety Committee and may involve 
suspension of RAM use priveleges.
So far, we have been able to correct problems before the Committee has to 
become involved.

At  Ohio State University, we are looking at the point system for 
conducting
laboratory inspections and evaluating Approved Supervisors' radiation 
safety
programs. I need information from other universities that use a point
system, particularly what action  is taken when certain number of points 
are
accumulated by the Approved Supervisor. Is there a suspension of privilege
to use radioactive materials if a certain number of points are exceeded? Is
the suspension temporary? What constitutes permanent suspension?
