Just to add some numbers to Jim's comments:
A few years ago I did a cursory review on this topic using data
collected by the Texas BRC's Incident Investigation Program. Texas took
the lead and mandated 2 person crews and formal training requirements
around 1986. The number of reported overexposures in Texas basically
took an immediate 50% decrease (looked like a step function). While I
did not analyze dose, per se, it was noted that direct health effects
(e.g., amputations, severe burns, etc...) almost completely disappeared.
That rulemaking is just about the classic case for demonstrating
positive health effects from regulation. The only comparable example I
can come up with are the XRF guidelines that came out in the 60's based
on the work in Pennsylvania (generally adopted by States, Users and
Manufacturers).
I would also mention the CRCPD's NEXT and DENT (funded by FDA, I
believe) studies on x-ray equipment exposures which lead to
significantly lowered patient dose (but may or may not have demonstrated
a decrease in direct health effects).

Wes is also correct that several of the states, among others,
were against the mandated use of EPDs in radiography based on their
regulatory experience. Manditory certification of radiographers has
had more positive impact on the safety. Also, the requirement for two
person crews has also provided an improvement in safety. In my
experience we have come a long, long way in radiograpy safety thanks to
these requirements.
