NRC, "Uniform Low-Level Radioactive Waste Manifest"

The NRC has issued new versions of the "Uniform Low-Level Radioactive Waste 
Manifest" (NRC Forms 540, 540A, 541, 541A, 542, and 542A), along with a new 
revision of NUREG/BR-0204, "Instructions for Completing NRC's Uniform 
Low-Level Radioactive Waste Manifest."  These may be obtained from the NRC
by 
calling Beverly Martin, at 301-415-5877, or e-mail bam1@nrc.gov.
