Looking for a Nuclear Chicago contact

I have an old 25 mCi Cs-137 Nuclear Chicago level gauge that is no longer 
needed.  I'm trying to find a phone number for Nuclear Chicago to see if they'll
take the source back.  Does anyone know how I can reach them or if they are even
still around?
