It could also be a coworker who sits next to the patient who "can't miss anymore work" because of all the sick days he/she has taken recently.  Get dosed in the morning, go straight to work.  How many people can say they do exactly as the Dr. says as soon as they leave the office?  In the case of the industrial radiographer it is the licensee who is responsible for seeing that the regulations are followed, with the patient the licensee has no control. ( I know radiography isn't the best example when it comes to compliance.)  Other problems could come from a patient urinating in a public restroom for the first void or vomiting in a public area.  It really seems that defending this decision will be trouble.
