AW: [ RadSafe ] Central Dose registry

How do these various national registries differentiate between people with the same first and last names, i.e. do they have a national identification number, etc?  Presuming the use of a unique identification number, how is the number kept confidential?  Do most US facilities utilize workers Social Security numbers, or do they have a unique in-house identification numbering system?  Is there a US governmental organization that consolidates information from the various private Dosimetry Corporations and public institutions that have their own Dosimetry programs, and maintains a national registry?
