It is good to see that this points at the method:

The method is to send 20 page questionnaires to cases and controls. After 
two reminders a follow up is instead done by telephone giving a 90 % 
response rate. How on earth do you make balanced interviews with hundreds of 
people (more than 500?) to get answers corresponding to 20 pages of 
questions?

If the method is wrong it doesn't matter what the statistics say because the 
methodological problem will then dominate.
