Latha,

I'll answer for how we utilize client information and what options we provide:

1. SSN is not required
2. Any unique identification can be used that distinguishes an individual from another that has same last name, first and even middle initial
3. All reports we send can suppress the SSN field (any field actually) including all electronic files sent
4. As far as Form 4 and Form 4 (NRC and states) they simply want facility to be able to correlate a SSN with an individual's reports
5. All reports in future will include just the last 4 digits, as one sees on bank records, etc.
6. Bottom line, one need not use the SSN. The facility simply needs to be able to determine which records pertain to which individual
