Latha -

Not only are SSNs not required to be used as ID numbers on dosimetry reports, they definitely should not be used, in order to limit the potential for identify theft.  While Landauer still wants SSNs for their records, you can instruct the company not to list SSNs on their reports, and have them use some other unique ID no. that you provide them  I have seen companies use driver license numbers, but I prefer company-issued ID numbers.  Whatever you use as an alternative to SSNs, all you need to do is list "OTH" in Item 3 of Form 4/5, as allowed by the instructions included on the back of the forms.  Equivalent forms provided by the state of Texas, and other Agreement States also allow use of "OTH" ID numbers.

Hi team:
I know this has been addressed in the past years. but i kinda missed the final outcome. Landauer still uses the SS# for personnel on their records. NRC form 4/5 uses the ss# . for filing termination reports and also the annual reports for personnel, how are you all handling the paperwork. how are you all storing the info for security reasons?
are we required to enter the ss # in the forms? can we just keep track with the partcipant no. as opposed to ss #.
just wanted to get a feel how other facilities handle this.
