NRC Information Notice on Medical Misadministratios

The NRC recently issued Information Notice 2000-05, "Recent Medical
Misadministrations Resulting From Inattention To Detail."  This is
available on the NRC Web site, and I recommend that all hp's read it.

Although the cited incidents are limited to medical misadministrations,
the root causes are generic.  They are primarily:  not following
procedures, not obtaining independent validation for important steps,
and not stopping to reassess the process when an unexpected response
occurs.
