Title: What's a record?

I have a question regarding byproduct materials license requriements. Pursuant to 10CFR30.51 "the licensee shall retain each record of receipr of byproduct material as long as...." I would like to know what constitutes a record of receipt. Is it the shipping papers, the initial leak test papers from the manufacturer, or all information received with the source?
