Fluoro Accident???

Can anyone point me to some information regarding a Fluoro accident -

recent - where the patient got radiation burns?



I am assuming "recent" means the past 5 years and I forget which state

it was supposed to be in - but its and eastern state.



I'd like to know the details of the accident and the equipment involved.
