Here I need to point out that admin and

engineering controls were violated by the

administrator/manager.............

The operators had NO CHOICE other than to fold

down under the pressure from the administrator or

to be replaced immediatelly.

I've sent.  'Apologies for the delay.

1.  Chernobyl.  Not only were the admin controls

defeated, so were some of the engineered

controls.  Even the best designs are vulnerable

to intentional acts.

This is the inherent weakness of administative

controls - they can be defeated.  We not only

have to assure that public that our safety

culture does not permit such practices, but also

provide engineered protection against credible

screwups.  Being safe 99.99% of the time isn't

good enough.
