(2) I don't "want to get irate" over anything. I just believe that when there is a clear trend of recurring events, business as usual isn't good enough.

If you want to get irate over some serious accidents, not just simple violations, then look to the x-ray overexposure and misadministration events, because that's where you will generally find actual life and health threatening events. Although, because we do not have a federal program, like the NRC, tracking and trending these events, they may be hard to identify.
