This is very true and part of our problem with
perception of bioterrorism, which is also related to
our fear of radiation, chemical weapons, etc.
When epidemics, like SARS, are unique, the public
health system has to first recognize there is a
problem. Once it is, it is no longer a surprise
attack. The anthrax attack was not noticed initially
since the last large case in in the 1960's. With our
heightened vigilance, biological attacks will be
harder to pull off.
And, as you point out, with virulent diseases like
smallpox, Eboli, etc., the victims are acutely ill and
do not travel well.
