Subject: PWR Fuel Damage



The NRC sent out an information notice yesterday, IN2004-04, titled

"FUEL DAMAGE DURING CLEANING AT A FOREIGN PRESSURIZED WATER REACTOR" in

which they describe damage to PWR fuel during cleaning at a "foreign PWR

site".  See

They described the releases as 10,000 Curies of Noble gases and 10

curies of radioiodines.



Can anyone tell me where this occured and possible further references?
