E-mail Addresses

I'll have to correct my previous, SOME email addresses did 
make it (more than in any previous edition for sure). They 
just have the wrong one for me!  But it is there and there 
are more than in the past, but the hundreds that don't are 
still curious!  Perhaps we need a better entry point on the
dues form like the AAPM does?
