I have generally had good service from Landuaer.  Directly or
indirectly I deal with about 20 facilities using their badges. 
That's somewhere in the range of 3,000-4,000 badges.  Under ALARA
we chase down a lot of Level I people ( > 125 mrem/qrt).  These are
some of the things I've identified,

- badges were held in a desk awaiting collection and shipment,
unknown to individual, a therapy dose was set on the desk for 15
minutes while tech went to the boys room.

- secretary collecting and holding badges in one department was
recieving I-131 therapy and didn't consider this of issue.

- badges were exposed to heat while being taken from one facility
to another, individual stopped for lunch on a sunny day and left
badges in direct summer sunlight for 45 minutes on the truck dash.

- individual kept badges in a black pottery dish which was found to
have uranium oxides.

I'm sure there are others and I know it's a pain, but its not
always the badge service.
