     MILLSTONE 2.  On August 8, 1995, operators manually
tripped Millstone 2 from 60% power.  The station had been
having seal problems with both the A and B heater drain tank
pumps.  Operators had previously taken the A heater drain
tank pump out of service for seal replacement and reduced
power to 80%.  Because of seal problems with the B heater
drain pump, operators reduced power to 60% and were taking
the B pump out of service.  After tripping the B pump,
operators noted a large steam leak in the area of the pump.
     Operators manually tripped the reactor and turbine
after identifying a non-isolable rupture in an 8"
recirculation line to the heater drain tank.  They closed
MSIVs, isolated heater drains, and secured the secondary
plant during post-trip response.
     During the transient, one safety valve (1000 psi
setpoint) on the B steam generator lifted (970 psi) and
failed to repeat for 5-10 minutes.  The plant was stabilized
20 minutes after the trip.
     14-hours later with the reactor in hot standby, the No.
2 atmosphere dump valve (ADV), which was being used to
control RCS temperature, failed open.  Flow from the ADV was
isolated manually in 20 minutes.
     The line rupture, a 12" long tear, was located midway
in a straight vertical 20' run of pipe 3" downstream of a
closed manual valve.  The line is in the plant's Erosion/
Corrosion Program.  Post-event measurements revealed wall
thickness of 0.041" at the tear.  Nominal wall thickness is
0.322" and the predicted wall loss was 0.075", so this
portion of piping was not identified as critical and was not
expected to experience the observed wall thinning.

IS TEAMWORK REALLY NECESSARY?
"Although teamwork is crucial to the success of many high-
performance work systems (where people work in a tightly
coordinated fashion and share responsibility for an entire
business process), the role of teamwork is changing as
organizations move toward the "virtual workplace" -- in
which people work at different times and places. Instead of
team play, the emphasis will be on individual performance.
More specialists will be required in the virtual workplace
than in the high performance organization (though this is
not a return to traditional divisions of labor).  Instead,
the specialists will be available on an on-demand basis, by
phone or network.  There will be a trend toward less
reliance on carefully scheduled and managed teamwork, and
more emphasis on individual contribution.  Direct labor
costs will be lower in the virtual workplace than in the
high-performance organization because more flexible work
arrangements will replace permanent employment... "We will
still have teams in the virtual workplace, but they will not
have the identity and prominence of high-performance work
