     Just a comment on the RSO magazine article mentioned on this board as 
     a reference for DPW issues.  The article refers to Reg Guide 8.13 for 
     a form that can be completed as a declaration form.  But the only Reg 
     Guide 8.13 that has that form is a draft version.  It is also my 
     understanding that the draft Reg Guide is undergoing significant 
     additional review (due to comments, etc.,) before it is issued as a 
     final revision.  Thus, the final NRC recommended form may be different 
     or it may be eliminated altogether from the draft.
