The methodology noted in the post,

is found on page 34 in NCRP Report 122, "Use of Personal Monitors to 
Estimate Effective Dose Equivalent and Effective Dose to Workers for 
External Exposure to Low-LET Radiation" Dec. 27. 1995.   It is an 
empirically derived formula.  Use of this formula is noted as unique to 
workers receiving radiation from medical x-rays, particularly 
fluoroscopy.  It is a modification of the work of Faulkner, K. and 
Harris, R.M. (1988) "Estimation of effective dose equivalent to staff in 
diagnostic radiology"  Phys. Med. Biol. 33, 83-91 and Webster, E.W. 
(1989) "EDE  for exposure with protective aprons" Health Phys. 56, 
568-569.
