I had a Windows based program taylored for my facility made up by 
VISTA Enterprises in Palmero,ME.  The nice thing about this program
is that we put in pictures of our facility, instruments, etc.  It
also allowed me have the vendor put in specific questions about our
facility.  It allows me to keep track of training records and the
vendor will update the program for a nominal fee.  The program is
now running on three servers.  The cost was US$6,000.
