I know this might be fundamental, but have you asked him where he kept his
ring badge when he wasn't wearing it.  I've seen large ring and badge doses
before that were the result of improper storage in radiation areas (although
not nearly as high).  This is sometimes hard to prove, but it's worth
investigating if you haven't looked at it already.  

Also, is this person involved with retrieval of the I-125 from the reactor? 
Is there a possible exposure pathway in that process?
