"Medical Management of Radiation Accidents" from the CRC Press is an
excellent book on the subject.  Although the primary focus is on the
mangement and medical care of victims, there are a few chapters addressing
specific accidents, including "Historical Aspects of Radiation Accidents,"
"Military Radiation Accidents," and "The Three Mile Island and Chernobyl
Reactor Accidents."  One of the authors is Dr. Robert Ricks from REAC/TS
in Oak Ridge, where they have extensive records on radiation accidents.
