Sometimes and accident is truly the result of not knowing something. 
When the wings came off the Electra airplanes, humans didn't know about
the "whirl mode" that could happen in that plane.  However, I agree that
most accidents are caused by attitude and nothing else.  Why else would
otherwise rational humans drink and drive and kill themselves and
others?  We all know that drinking is the cause of inattention and all
of the other things that lead to accidents by drinking drivers.  But it
is the attitude of the driver that causes the accident, whatever that
attitude is.  Until we learn how to get people to change their
attitudes, nothing we do will prevent accidents.  Al Tschaeche
