Location or information about Nuclear Chicago

    We have a Nuclear Chicago depth density gauge whch has not been used
for over ten years and now a researcher would like to take it on a long
trip.  I need to find out who would have or know the location of
information for a special form certificate.
The instrument in question is a

Nuclear Chicago depth density gauge
    P20 and a P205 probe with an activity of 111 MBq of Cs-137
