Shipping Violation

I have a question on a shipping violation.  Our organization just received a
limited quantity shipment.  This shipment included an internally
contaminated vacuum pump.  According to the documents the pump contains 9
Ci. of H3.  The way I read the regs. this pump should have been shipped in a
type A container.  The question is does this require notification to the
NRC.  I have been unable to find any requirement for notification since no
exposure or spread of contamination was involved.  If someone feels I am
misreading or missing the requirements in the regulations please let me know.
