Q: internal radiation permits

We're in the process of revamping the internal permits we issue to
radionuclide users.  It's about time we subjected our probably
decades-old permit format to some (pardon the expression) "continual
improvement."

It would really help us to look at other internal permits, particularly
for large research universities with broad scope rad mat'l licenses. 
Examples (with any sensitive information removed, of course) would be
greatly appreciated, particularly in regards to the wording of
conditions of use, etc.  I am also planning to make permit generation
database-driven, so any tips on making the format amenable for that
purpose would also be appreciated.  Any examples we receive will be for
internal reference only and will not be shared.
