Ran into the same problem years ago.  Found out that it was the cap.  I
remember the cap had a foil in it for a seal and apparently that caused
some sort of problem (he manufacturer was aware of the problem, but I
never did find out what caused it).  Try a different manufacturer's vile
and cap.
