I've had very good luck using the program "Digital Trainer."  I have version 4 and will be

getting version 5 Professional in a week or so.  The "professional edition" has all of the

functionality you are looking for.  The learning curve is relatively short, so all that
really needs to be done is plugging in the questions.  You can visit their web site at

Of course (ahem) if you're looking for a contractor to put this all together for you (the
software license does permit me to redistribute finished products so there would be no
need to re-purchase the software for your facility), please contact me.  It would probably
cost less than the purchase of the software.
