? Lifetime EDE

I have a question that can be answered by state regulators, and,
hospital facilities, where EDE records are maintained (either using
standard Webster dose weighting, or other weighting method).

(1) Is EDE lifetime maintained in the dose history records (either
for single or double badging?

(2) If EDE lifetime is maintained, how do you meet the requirements
of providing an employee request for all dose information upon
termination? Do you breakout the EDE information from the normal
monitoring, or, do you combine the former WB, DDE and EDE (based on
the time period for the exposures received?

(3) How do you define YTD on the annual NRC Form 5 Equivalent when
there has been an EDE calculation?

Any information would be appreciated.

Thanks...
