particular, what limits do you currently use to determine when to perform a
direct or indirect leak check? For example, do you use a certain exposure
rate to determine if the technician performing the swipe should perform an
indirect leak check vs. direct leak check?

Check out ANSI/HPS N43.6-1997, "Sealed Radioactive Sources - Classification"
Annex A, "Leak-test Methods." I could fax the applicable pages to you if
you do not have a copy.
You asked:
2) What sort of guidance are you using for when to use extremity dosimetry
for handling sources?
Reply:
Try DOE Guide 441.1-4, "External Dosimetry Program Guide" and HPS
N13.41-1997, "Criteria for Performing Multiple Dosimetry." Again,
definitely faxable.
