You should consult the NRC's Federal Register notice of January 29, 1997.
It addresses most of the issues
you raise.  To get a copy of the notice use the WWW address  of the (US)
Government Printing Office "http://www.gpo.gov"; and do a search of the Fed.
Reg. of that date for something like "nuclear patient"which should only
produce one hit.
