Steve, you are correct about having an initiating event, but none that I am aware of
ever involved the death of anyone in the public due to radiation releases.  SONGS
had one drill involving a spent fuel element or refueling bridge crash.  The operator
may have been killed, but I do not remember if it was by delayed actions or a
physical injury.
