I think Mr. Potter may have hit the nail on the head. Too often we are driven by internal or external forces to achieve lower NUMBERS (dose, contamination incidents, rad waste volume) which may or may not reflect an excellent program or excellent performance (I think you can easily have an excellent performance without an excellent program, and vice versa). For example, a reduction in the hallowed numbers does not necessarily mean your workers are well trained, know how to prevent or respond to incidents, have a questioning attitude, properly obtain, record and report data, etc. Look at the performance indicators provided in DOE's RadCon Manual. Does anyone doubt that one could (some have?) pursue these good numbers without developing "excellent" programs? The fact that doses and contamination incidents are low does not mean you are not within an inch of a severe incident.
